Department of Technology Request Form - Please use the Helpdesk Request system for your request entry.

Department of Technology User ID Request Form - (Revised 8/26/2009) this form is used to request an account on the Mainframes.  The request must be approved by users Supervisor and campus Principal or Asst. Superintendent (as appropriate), you must also have the appropriate signatures from Payroll and Personnel.   The form must be delivered or mailed to Information Systems.  The User ID Request form is in Adobe's PDF format.  Use Adobe Acrobat  Reader to print this form.   PLEASE DISCARD ALL OTHER VERSIONS!

Note:  Supervisors for new users will need to also submit a HelpDesk request for a computer network login account, e-mail account, printer setup, and installation of any programs pertinent to your department to be installed by an Department of Technology's Computer Specialist.

Training will be provided for the new employee when requested by their supervisor via a HelpDesk request.

Computer System Use Policy Form - this form must be submitted along with any new request for accounts. Please provide all information requested.  The Computer System Use Policy form is in Adobe's PDF format.  Use Adobe Acrobat  Reader to print this form.
 

Department of Technology's Reactivation Form - 11-12 - this form is used to reactivate an account after it has been deactivated.  The reactivation must have an immediate supervisor's authorization and must be delivered or mailed to Department of Technology.  You must have your log-on on this form. i.e.(NEWUSER,AT001.STUDENT) or (Skyward,AAAAAAAA000)    The Reactivation form is in Adobe's PDF format.  Use Adobe Acrobat  Reader to print this form.   PLEASE DISCARD ALL OTHER VERSIONS OF THIS FORM
 

 

 

Department of Techonology's PDA Usage Policy and Agreement Form - this form is to define standards, procedures and restriction for connecting to McAllen ISD's internal network.
ie: Access to your District e-mail.
